FOXO4-DRI 10 mg vial dosage

A 10 mg FOXO4-DRI vial mixed with 1 mL of bacteriostatic water gives 10 mg/mL, so 2.5 mg is 25 units (0.25 mL) on a U-100 insulin syringe. At 2.5 mg a dose, one vial holds 4 doses.

What FOXO4-DRI is

Class:
Synthetic D-retro-inverso peptide (FOXO4-p53 interaction inhibitor)
Also called:
FOXO4-DRI peptide, FOXO4 D-retro-inverso peptide
Used for:
Sold online as a senolytic research chemical for anti-aging use. Designed in the laboratory to disrupt an interaction between the FOXO4 and p53 proteins inside senescent cells; not studied in humans.
Human evidence:
Animal / lab only. A PubMed search for FOXO4-DRI (queried 24 September 2026) returns 19 records and zero indexed under a clinical-trial or randomised-controlled-trial publication type. The public record consists of cell-culture and rodent work; I could not identify any published human study of FOXO4-DRI by any route.
US status:
Research use only. Not an FDA-approved drug and not a dietary supplement. FOXO4-DRI does not appear in the Category 2 table or the withdrawn-nominations table on FDA's significant-safety-risks compounding page (checked 24 September 2026). That page does not cover every FDA nomination category, so this does not establish whether FOXO4-DRI has been nominated elsewhere in the 503A Bulks List process. It is sold to consumers as a research chemical, a labelling choice rather than a legal category.

How to mix a 10 mg FOXO4-DRI vial

For this vial: 1 mL of bacteriostatic water makes 10 mg/mL, so 2.5 mg is 25 units (0.25 mL) on a U-100 insulin syringe. Pick a different volume from the units table if you want bigger or smaller draws; the number of doses stays the same.

  1. Wipe the rubber stoppers of both the peptide vial and the bacteriostatic water vial with an alcohol swab and let the alcohol dry. [4][5]
  2. With a new sterile needle and syringe, draw up the amount of bacteriostatic water you have chosen for the dilution. [4][5][6]
  3. Inject the water into the peptide vial by aiming the stream against the inside glass wall, which helps prevent foaming. [7]
  4. Swirl or roll the vial gently until the powder has fully dissolved; do not shake it. Do not inject the solution if it is cloudy or has particles in it. [4][5][7]
  5. Label the vial with the date and time it was mixed, the amount of water added and how long it can be used after mixing. [4]
  6. Store the mixed vial in the refrigerator at 2-8°C (36-46°F) and do not freeze it. [7]

Step by step with pictures: how to reconstitute peptides. About the water itself: bacteriostatic water.

Storing FOXO4-DRI vials

  • Freeze-dried (lyophilized) peptide keeps far longer than peptide in solution; peptide suppliers advise storing the powder at -20°C away from bright light. [8]
  • Protect peptide vials from heat and light. [8][9]
  • Avoid repeated freeze-thaw cycles; a prescription peptide label also says not to freeze a vial once it has been mixed. [7][8]
  • Once a multi-dose vial, such as bacteriostatic water, has been opened (needle-punctured), date it and discard it within 28 days unless its manufacturer states another date. [10][11]

More detail: peptide storage and shelf life.

Injecting safely

  • Clean the injection site with an alcohol swab and let the skin dry completely before injecting. [12][13]
  • Pinch up a fold of skin and fat (not muscle) and insert the needle at a 45- to 90-degree angle: 90 degrees where there is enough fatty tissue, 45 degrees where there is little. [12][13][14]
  • Pulling back on the plunger (aspiration) is not necessary for a subcutaneous injection, because the chance of the needle being in a blood vessel is small. [12]
  • Inject slowly, at roughly 10 seconds per mL. [12]
  • Common sites are the belly, the thigh and the upper arm; rotate sites, keeping each injection at least 1 inch (2.5 cm) from the last. [7][12][13]

Hygiene and warning signs

  • Use a new sterile needle and syringe for every injection and every time you enter a vial; never reuse them. [6][10][13]
  • Never share needles, syringes or pens with anyone, even if the needle is changed; unsafe injection practices spread hepatitis B, hepatitis C and HIV. [6][9][10]
  • Put used needles and syringes in a sharps disposal container straight away (FDA-cleared if possible), never loose in the trash, recycling or toilet, and dispose of the container following your community's guidelines. [15][16]
  • Contact a health care provider if the injection site shows signs of infection, such as redness, swelling, red streaks or a fever. [13]

Where to inject: subcutaneous injection sites.

How FOXO4-DRI works

  • FOXO4-DRI is a peptide built from mirror-image amino acids in reverse order, designed to get in the way of the interaction between FOXO4 and the tumour-suppressor protein p53. In senescent cells that interference pushes p53 out of the nucleus and triggers the cell's own apoptosis; the researchers identified FOXO4 as the pivot on which senescent cell survival turns. [17]
  • Structural work since has shown what it binds. Solution NMR models place FOXO4-DRI on the disordered transactivation domain of p53, forming a transiently folded complex to which both the FOXO4-derived sequence and the positively charged cell-entry tail contribute, and phosphorylation of p53 increases its affinity. [18]

What the FOXO4-DRI research found

Human evidence: animal / lab only. Each result says what was studied.

  • Animal studyIn the 2017 paper that introduced it, the peptide counteracted doxorubicin chemotoxicity in mice and restored fitness, fur density and kidney function in both fast-ageing XpdTTD/TTD mice and naturally aged mice. [17]
  • Animal studyIn a hydrogen-peroxide model of senescent mouse Leydig cells, FOXO4 kept the senescent cells alive and FOXO4-DRI selectively triggered their apoptosis. In naturally aged mice, the peptide improved the testicular environment and eased the age-related shortfall in testosterone secretion. [19]
  • Lab studyTested on human cells rather than rodents, the peptide removed more than half the cells from chondrocytes expanded to nine population doublings while leaving minimally expanded cells largely alone, and lowered senescence markers. It did not improve the cartilage-forming ability of the surviving cells, which was the point of the experiment. [20]
  • Animal studyIn mice given bleomycin to induce lung fibrosis, treatment produced milder pathology and less collagen deposition than bleomycin alone, alongside a shift in where p53 sat within the nucleus and less extracellular matrix protein overall. [21]
  • Animal studyA result that cuts the other way: in mice overexpressing the serotonin transporter, FOXO4-DRI - like the senolytic drug ABT263 - produced worsened pulmonary haemodynamics and a loss of pulmonary endothelial cells. The authors concluded that clearing senescent pulmonary endothelial cells may worsen pulmonary haemodynamics, and that senolytic strategies should be weighed for their effect on the lung's blood vessels. [22]

FOXO4-DRI side effects and risks

No published human study of FOXO4-DRI exists, by any route, so there is no human safety record at all. Everything below is animal or cell work.

  • Animal studyThe one clear harm signal in the published record is the pulmonary hypertension work: eliminating senescent cells with FOXO4-DRI in mice was associated with adverse pulmonary haemodynamic changes and depletion of pulmonary endothelial cells, in a model where senescent endothelial cells appear to be doing something useful. [22]
  • Lab studyThe mechanism itself is the open question. The peptide works by binding the transactivation domain of p53 to force it out of the nucleus, and the structural paper presents that interaction as a basis for developing p53 inhibitors. What repeatedly interfering with a tumour-suppressor pathway does in a person over time has not been studied, because no human study exists. [17][18]
  • Lab studySelectivity for senescent over healthy cells holds up in a dish but is not absolute or automatic: in expanded human chondrocytes the peptide removed more than half of the heavily expanded cells and left the minimally expanded ones broadly intact, which is a difference in degree measured in one cell type, not a demonstration that healthy tissue is unaffected in a body. [20]

Other things to know

  • No published human study exists, so there is no human safety record of any kind for FOXO4-DRI.
  • No human pharmacokinetic data exist. Any dosing figure circulating online is not built on a measured human number.
  • The senolytic mechanism is a laboratory hypothesis, not a demonstrated effect in people; selectivity for senescent over healthy cells has not been confirmed in humans.
  • Not FDA-approved, not reviewed by FDA for compounding, and not legally marketed for human use.

Questions

How much water do I add to a 10 mg FOXO4-DRI vial?
There is no single correct amount: water changes how many units a dose is, not how many doses the vial holds. With 1 mL the vial is 10 mg/mL and 2.5 mg is 25 units (0.25 mL) on a U-100 insulin syringe. The units table gives 1, 2, 3 mL.
How many doses are in a 10 mg FOXO4-DRI vial?
40 doses at 250 mcg, 4 doses at 2.5 mg, 2 doses at 5 mg.
How long does a 10 mg FOXO4-DRI vial last?
At 2.5 mg a dose and reported protocols vary widely - some use daily dosing, about 0.6-1.3 weeks. How long a mixed vial keeps is a separate limit; see storage below.
How many 10 mg vials do I need for a cycle?
1 vial at 250 mcg 3 times a week for 2 weeks; 56 vials at 5 mg 7 times a week for 16 weeks.
What does 1 unit equal in a 10 mg FOXO4-DRI vial?
100 mcg with 1 mL, 50 mcg with 2 mL, 33.33 mcg with 3 mL of bacteriostatic water, on a U-100 syringe.
