Epitalon: One Telomere Study, in Cell Lines, Doing All the Work
Epitalon has 142 papers, one tagged clinical trial and no randomised ones. The telomere result everyone cites was in cultured cells, not people.

Epitalon is a four-amino-acid peptide from the same Russian research programme as Semax and Selank. It is sold for longevity on the strength of telomere research, and the headline telomere result was measured in cultured human cell lines rather than in people.

Epitalon — often spelled epithalon, which is the spelling most people search — is sold as a longevity compound on the back of one idea: it lengthens telomeres.
That claim has a real study behind it. The study is in cultured cells.
What the literature contains
Epitalon comes from the same Russian peptide-bioregulator programme that produced Semax and Selank Khavinson 2002 - Khavinson VKh. Peptides and Ageing. Neuro Endocrinol Lett. 2002;23 Suppl 3:11-144. (opens PubMed in a new tab), and the same caveat applies without needing to be re-argued: research published in Russian is still research, it has not been independently replicated outside that tradition, and you cannot easily check it. What is different here is how little clinical work there is even inside that tradition. One tagged clinical trial, and nothing randomised at all.
The telomere study, read carefully

And the step before that one
Even granting the mechanism entirely, there is a prior question the marketing skips: is longer telomere length good for you?
The relationship between telomere length and health is genuinely contested. Telomere shortening is associated with aging, but lengthening them has not been established as a health benefit, and the biology has complications that make “longer is better” an unsafe assumption rather than an obvious one.
So the claim chain runs: a laboratory measurement, in cells, of a marker whose relationship to health outcomes is unresolved. Every link in that chain is doing work, and the marketing presents the whole thing as a conclusion.
Dose
No completed randomised trial establishes one, and we are not transcribing numbers out of the Russian-language literature we cannot properly appraise. That is the same standard applied on the Semax and Selank page and for the same reason.
