Tesamorelin Dosage Chart: The FDA Label Dose and Reconstitution
Tesamorelin dosage chart: units per dose by vial size, the Egrifta WR label dose (1.28 mg SC once daily), reconstitution steps, and when to stop.
The short version
Tesamorelin is dosed exactly as the FDA-approved Egrifta WR label states. That is 1.28 mg injected under the skin of the abdomen once daily, from a vial reconstituted with the supplied diluent and used within 7 days. It is prescribed for a specific condition, HIV-associated lipodystrophy. The label lists who should not take it and when to stop. Nothing here is a protocol for using it outside that approval.
- The only dose is the label dose: 1.28 mg subcutaneously, once daily, in the abdomen.
- One reconstituted vial covers 7 daily doses, then must be discarded.
- The label excludes active malignancy, pituitary-axis disruption and pregnancy, and lists when to stop.
Tesamorelin dosage chart
The FDA-approved Tesamorelin product is dosed as its label directs and a prescriber sets the dose. For vials that need mixing, this is each label dose in units on a U-100 insulin syringe, by vial size.

Tesamorelin is sold as Egrifta WR, an FDA-approved prescription medicine. This page gives the dosing exactly as the current label does. It is not a protocol and does not extend to any use the label doesn’t cover.
To convert a vial’s concentration into a syringe volume, use the dosage calculator rather than doing the maths by hand.
Tesamorelin dosage: the approved dose
| Dose | 1.28 mg |
| Route | Subcutaneous injection |
| Frequency | Once daily |
| Site | Abdomen, rotating injection sites |
| Reconstitution | 1 vial of lyophilized powder with 1.3 mL of the supplied diluent, swirled (not shaken) |
| Vial life once mixed | 7 daily doses, stored at room temperature (20-25°C / 68-77°F), then discarded |
That is the whole approved regimen. The label has no titration schedule and no alternate dose, because the approval covers one population: adults with HIV-associated lipodystrophy and excess visceral abdominal fat.
Who should not use tesamorelin
The label lists tesamorelin as contraindicated in:
- Patients with disruption of the hypothalamic-pituitary axis, from any cause (for example, hypophysectomy, pituitary tumour, radiation, or trauma).
- Patients with active malignancy.
- Patients with known hypersensitivity to tesamorelin or the product’s excipients.
- Pregnant women. The label states that modifying visceral fat offers no benefit during pregnancy and could cause fetal harm.
Tesamorelin side effects and risks the label describes
- Increased risk of neoplasms. Tesamorelin raises endogenous growth hormone, a growth factor. That matters in a patient group that may already have or develop malignancies.
- Elevated IGF-1. The label notes the effects of prolonged IGF-1 elevation are unknown.
- Fluid retention: oedema, joint pain and carpal tunnel syndrome, linked to growth hormone induction.
- Glucose intolerance or new diabetes. The label directs you to evaluate glucose status before starting and monitor it periodically during treatment.
- Hypersensitivity reactions, reported in 4% of patients on tesamorelin in clinical trials.
- Injection site reactions, and an increased mortality risk associated with growth hormone in acutely critically ill patients.
When to get emergency care
Signs of a serious allergic reaction are a medical emergency. These include swelling of the face, lips, tongue or throat, difficulty breathing, or a rash spreading quickly. In the US, call 911. For a non-emergency question about a reaction, or about a possible exposure, Poison Control is on 1-800-222-1222.
US legal status - FDA-approved · prescription only
Tesamorelin (Egrifta WR) is an FDA-approved prescription medicine. It requires a prescriber, and the dosing above is theirs to manage against your bloodwork - this page describes the label, not a recommendation to use it.
Where to get Tesamorelin legally
Tesamorelin is an FDA-approved prescription medicine, so the legal route is a licensed prescriber, in person or by telehealth, and a licensed pharmacy.
I found no published cash price for EGRIFTA; the maker's THERA programme supplies it free to qualifying patients.

These checked vendors sell a research-labelled version. It has not been reviewed by FDA, is labelled not for human use, and is tested only as the seller chooses.

Common questions
What is the tesamorelin dosage?
Per the current FDA label for Egrifta WR, the recommended dose is 1.28 mg injected subcutaneously once daily, into the abdomen, with injection sites rotated. That is the only dose the approval covers.
How is tesamorelin reconstituted?
Per the label, one vial of Egrifta WR lyophilized powder is reconstituted with 1.3 mL of the supplied diluent and swirled (not shaken) to mix. One reconstituted vial provides daily doses for 7 consecutive days at room temperature, then is discarded.
Does the tesamorelin dose change for weight loss in people without HIV?
No dose exists for that use. The 1.28 mg dose was established for reducing visceral abdominal fat in HIV-associated lipodystrophy, the only approved indication. No trial has set a dose for healthy adults, and the label does not cover that use.
