Semaglutide Dosage Chart: The FDA Label Schedule, Step by Step

Semaglutide dosage chart: the FDA label schedules for Wegovy and Ozempic, with starting dose, titration and missed-dose rules.

The short version

Semaglutide is dosed once weekly, and the schedule differs by brand and indication. Wegovy titrates over 16 weeks to a 2.4 mg maintenance dose for weight management (the 2026 label allows up to 7.2 mg for some patients). Ozempic titrates over a shorter run to a maintenance dose between 0.5 mg and 2 mg for type 2 diabetes. Both come from the FDA-approved label, not from a forum or a vial with no prescriber attached.

  • Wegovy: 0.25 mg weekly for 4 weeks, then up every 4 weeks to 0.5, 1 and 1.7 mg. From week 17, 2.4 mg (or 1.7 mg); up to 7.2 mg after 4+ weeks at 2.4 mg.
  • Ozempic: 0.25 mg weekly for 4 weeks as an initiation dose, then titrated to a 0.5-2 mg weekly maintenance dose for glycemic control.
  • A missed dose has a specific rule on the label. It is not "just take it whenever you remember."
  • None of this applies to a compounded or "research use only" vial; that product has no FDA-reviewed dose at all.

Semaglutide dosage chart

The FDA-approved Semaglutide product is dosed as its label directs and a prescriber sets the dose. For vials that need mixing, this is each label dose in units on a U-100 insulin syringe, by vial size.

Semaglutide dosage is not one number. There are two titration schedules on two FDA labels, depending on brand and approved use. This page states both exactly as the labels give them.

For what semaglutide is approved for, its boxed warning and its full adverse-reaction profile, see the semaglutide guide.

US legal status - FDA-approved · prescription only

Semaglutide is FDA-approved as Ozempic, Wegovy and Rybelsus, each with its own label and dosing schedule below. It requires a prescriber, who titrates and adjusts the dose for an individual patient.

AU legal status - TGA-registered · prescription only

Semaglutide is Schedule 4 of the Poisons Standard - prescription only.

Regulators reach their own conclusions on their own timelines. Where these two disagree, the difference is the point - not an error.

Wegovy (weight management), weekly titration

WeeksDose
1–40.25 mg once weekly
5–80.5 mg once weekly
9–121 mg once weekly
13–161.7 mg once weekly
17 onward2.4 mg once weekly (recommended), or 1.7 mg if 2.4 mg is not tolerated
After 4+ weeks at 2.4 mgMay increase to a maximum of 7.2 mg once weekly if more weight reduction is needed

Adolescent patients (12 and older) titrate the same way to a 2.4 mg maintenance dose, with the same option to reduce to 1.7 mg for tolerability. If gastrointestinal side effects make a step hard to tolerate, the label allows delaying that dose increase for an additional 4 weeks before trying again.

Ozempic (type 2 diabetes), weekly titration

StepDose
Weeks 1–40.25 mg once weekly (an initiation dose - the label’s glycemic-control maintenance doses start at 0.5 mg)
After ≥4 weeks at 0.25 mg0.5 mg once weekly
After ≥4 weeks at 0.5 mg, if more control is needed1 mg once weekly
After ≥4 weeks at 1 mg, if more control is needed2 mg once weekly (maximum)

The label states no dose adjustment is recommended for renal or hepatic impairment. That is a general pharmacokinetic statement. It does not replace a prescriber reviewing an individual patient’s kidney or liver status.

How it’s injected

Both labels describe the same administration. Inject subcutaneously in the abdomen, thigh or upper arm, once weekly, on the same day each week, at any time of day, with or without meals. You can rotate the injection site freely within those regions without changing the dose.

Rybelsus (oral tablets) follows a separate, non-injectable dosing schedule on its own label, not covered by the injection instructions here.

Missed-dose rules

ProductIf it’s been…Then…
Wegovy≤48 hours until the next scheduled doseSkip the missed dose; resume the regular schedule
Wegovy>48 hours until the next scheduled doseTake it as soon as possible
OzempicWithin 5 days of the missed doseTake it as soon as possible
OzempicMore than 5 daysSkip it; resume the regular schedule

For 2 or more consecutive missed Wegovy doses, the current Wegovy label says to restart the dose escalation at a lower dose, to reduce the risk of stomach side effects. The Ozempic label gives no separate rule for several missed doses. The prescriber managing the treatment decides the restart dose, not you.

What is not established

There is no FDA-reviewed dose for semaglutide outside these labelled schedules. A “research” vial with dosing instructions printed on a sheet of paper has not gone through the review that produced the schedules above. In FDA’s own published language, it is being sold for human use outside any approval. Nothing on this page describes or endorses dosing such a product.

Who should not use semaglutide

Semaglutide is contraindicated for anyone with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. The boxed warning is covered in full on the semaglutide guide. The label also states that co-administration with another GLP-1 receptor agonist, or another semaglutide-containing product, is not recommended.

Semaglutide side effects during titration

Gastrointestinal effects (nausea, diarrhoea, vomiting, constipation) are the most common adverse reaction. They are most likely right after a dose increase, which is why the titration exists: to reach the effective dose without provoking them all at once.

The full adverse-reaction tables, and the less common but more serious risks, are on the semaglutide guide. Those risks are pancreatitis, gallbladder disease, kidney injury and retinopathy progression.

When to stop and get medical care

Seek emergency care (call 911 in the US) for any of these:

  • symptoms of a serious allergic reaction: swelling of the face, lips, tongue or throat; difficulty breathing; a fast heartbeat
  • severe or persistent abdominal pain that may signal pancreatitis
  • signs of a thyroid mass, such as a lump in the neck, hoarseness or difficulty swallowing

For an accidental extra dose, a suspected overdose, or any exposure that isn’t a clear emergency, call the US National Poison Control Center on 1-800-222-1222. It is free and confidential, 24 hours a day.

Where to get Semaglutide legally

Semaglutide is an FDA-approved prescription medicine, so the legal route is a licensed prescriber, in person or by telehealth, and a licensed pharmacy.

With a prescription, the Wegovy pill (1.5 mg) costs $149 a month self-pay from Novo Nordisk (as of 3 October 2026).

Research-labelled semaglutide (not the approved medicine)

These checked vendors sell a research-labelled version. It has not been reviewed by FDA, is labelled not for human use, and is tested only as the seller chooses.

Common questions

What is the starting dose of semaglutide?

Both Wegovy and Ozempic start at 0.25 mg once weekly for the first four weeks. On both labels, this starting dose is explicitly for treatment initiation and tolerability, not for glycemic control or weight management. The effective dose comes later in the titration.

What is the maximum dose of semaglutide?

For Wegovy, the recommended maintenance dose is 2.4 mg once weekly, with 1.7 mg as an alternative for patients who do not tolerate 2.4 mg. Since the June 2026 label, patients who need more weight reduction may go up to 7.2 mg once weekly. They must first have tolerated 2.4 mg for at least 4 weeks. That is the maximum. For Ozempic, the maximum recommended dose is 2 mg once weekly. The ceilings differ because the approved uses differ.

What happens if I miss a dose of semaglutide?

On the Wegovy label, if the next scheduled dose is more than 48 hours away, take the missed dose as soon as possible. If less than 48 hours remain, skip it and resume the regular schedule. Ozempic's label allows a missed dose to be taken within 5 days. After that, skip it and resume the regular schedule. If 2 or more consecutive Wegovy doses are missed, the Wegovy label says to restart the dose escalation at a lower dose to reduce the risk of stomach side effects. The Ozempic label sets no separate rule for several missed doses. The prescriber decides the restart dose.

Do I need to adjust semaglutide dosing for kidney or liver problems?

Ozempic's label states no dose adjustment is recommended for renal or hepatic impairment based on its pharmacokinetics. That is a general statement from the label, not permission to self-adjust anything. A prescriber managing chronic kidney disease or liver disease should weigh it for an individual case.
