Semaglutide: Cost, Uses, Dosage and Side Effects (Ozempic, Wegovy)
Semaglutide cost on 3 October 2026: $149 to $499 a month self-pay for Ozempic or Wegovy, $25 with covering insurance. Approved uses, label dosing and side effects.
The short version
Semaglutide is an FDA-approved GLP-1 medicine sold as Ozempic (type 2 diabetes, including heart and kidney risk reduction), Wegovy (weight management and heart risk reduction) and Rybelsus (oral, diabetes, now being replaced by Ozempic pill). As of 3 October 2026, Novo Nordisk sells it to cash payers for $149 to $499 a month, depending on brand, form and dose. Its label sets out a boxed warning, a dosing schedule and side-effect rates from the trials behind approval. None of that applies to a compounded or "research use only" vial sold under the same name.
- Self-pay semaglutide costs $149 to $299 a month as a pill and $199 to $499 as a pen (3 October 2026), or $25 with covering insurance.
- One molecule, three brand names, several FDA-approved uses.
- The label gives its boxed warning, dosing schedule and side-effect rates, so there is no need to guess.
- Grey-market semaglutide is a different supply chain with FDA-documented quality problems, not a cheaper version of the same product.

Cheapest semaglutide
The lowest manufacturer price is $149 a month, for the 1.5 mg Ozempic or Wegovy pill. That is a starting dose, used for the first 30 days. At maintenance doses the pills ($199 to $299) still cost less than the pens ($349 to $499). TrumpRx.gov and Costco Pharmacy listed the same self-pay prices as Novo.
With insurance or Medicare
- Commercial insurance: Novo’s savings offers bring Ozempic or Wegovy to as little as $25 a month, with at most $100 off a month. Government beneficiaries are excluded.
- Medicare, weight management: the Medicare GLP-1 Bridge charges $50 a month for every Wegovy pen and pill strength, from 1 July 2026 to 31 December 2027. It is for Part D members who meet its BMI and health criteria. It does not cover Ozempic or Rybelsus, and excludes people with type 2 diabetes.
- Medicare, type 2 diabetes: Novo says most Part D plans cover Ozempic, with the cost varying by plan.
What changes in 2027
Novo announced on 24 February 2026 that list prices will fall to $675 from 1 January 2027 for:
- the Wegovy 2.4 mg injection and 25 mg tablet;
- the Ozempic 0.5, 1 and 2 mg injection;
- Rybelsus 7 and 14 mg.
It says self-pay prices are not affected.

| Rybelsus | Daily tablet | Glycemic control in type 2 diabetes, and reducing major cardiovascular events in adults with type 2 diabetes at high risk |
| Wegovy | Weekly injection or daily tablet | Chronic weight management, and reducing cardiovascular risk in adults with established cardiovascular disease and obesity or overweight; the injection is also approved for noncirrhotic MASH with moderate to advanced liver fibrosis |
US legal status - FDA-approved · prescription only
Semaglutide is FDA-approved as Ozempic, Wegovy and Rybelsus, each with its own label. It requires a prescriber. A compounded, counterfeit or "research use only" vial is not the approved product and is not made to the same quality standard.
AU legal status - TGA-registered · prescription only
Semaglutide is Schedule 4 of the Poisons Standard - prescription only - and approved products are on the ARTG. Prescription medicines cannot lawfully be advertised to the Australian public.
Regulators reach their own conclusions on their own timelines. Where these two disagree, the difference is the point - not an error.

How semaglutide works
Semaglutide is a copy of the gut hormone GLP-1, with 94% of its sequence the same as human GLP-1. It binds and switches on the GLP-1 receptor, which sits in several brain areas that control appetite. It also raises insulin release and slows stomach emptying.
The label says the exact mechanism behind its cardiovascular benefit has not been established.
Semaglutide reviews: what the trials show
Star ratings on review sites come from people who chose to post. The trials in the Wegovy label give controlled numbers:
- Weight lost: in the main 68-week trial, weight fell 14.9% on 2.4 mg against 2.4% on placebo. In adults with type 2 diabetes it fell 9.6% against 3.4%.
- Higher dose and pill: at 72 weeks, 7.2 mg gave 18.8% against 15.5% on 2.4 mg. The 25 mg tablet gave 13.6% against 2.4% on placebo at 64 weeks.
- Stopping for side effects: in the weight-loss trials, 6.8% of people on 2.4 mg stopped because of side effects, against 3.2% on placebo, most often for nausea, vomiting or diarrhoea.
- After stopping: a year after the STEP 1 trial ended, people had regained 11.6 percentage points of a 17.3% loss Wilding 2022 - Wilding JPH, Batterham RL, Davies M et al. Weight regain and cardiometabolic effects after withdrawal of semaglutide: The STEP 1 trial extension. Diabetes Obes Metab. 2022;24(8):1553-1564., in the references below.

Semaglutide dosage
The approved schedule steps up slowly so the gut can adjust, not to reach the top dose fast:
| Weeks | Dose |
|---|---|
| 1–4 | 0.25 mg once weekly |
| 5–8 | 0.5 mg once weekly |
| 9–12 | 1 mg once weekly |
| 13–16 | 1.7 mg once weekly |
| Maintenance | 2.4 mg once weekly (or 1.7 mg) |
A prescriber adjusts this to your response and tolerance. It is not a schedule to run yourself from an unlabelled vial.
Semaglutide side effects
Everything here comes from the FDA-approved labels for Ozempic, Wegovy and Rybelsus, which report side-effect rates from the trials behind approval.
The boxed warning
The label carries a boxed warning, FDA’s strongest label warning, for thyroid C-cell tumours:
When starting
Gut effects are the most common, and the label says they increased while the dose was being raised. Selected rates from the trials behind the Wegovy label:
| Reaction | Semaglutide | Placebo |
|---|---|---|
| Nausea | 44% | 16% |
| Diarrhoea | 30% | 16% |
| Vomiting | 24% | 6% |
| Constipation | 24% | 11% |
Ongoing
Other reactions in 5% or more of patients, with no stated link to dose increases, include headache, fatigue, indigestion (dyspepsia), dizziness, bloating, and low blood sugar in people with type 2 diabetes. Combining semaglutide with insulin or a sulfonylurea raises the risk of low blood sugar, including severe episodes.
Rare but serious
- Pancreas. Acute pancreatitis, including fatal and non-fatal haemorrhagic or necrotising cases, occurred in 4 Wegovy-treated patients (0.2 cases per 100 patient-years) versus 1 on placebo in clinical trials.
- Gallbladder. Gallstones (cholelithiasis) occurred in 1.6% of adults on Wegovy versus 0.7% on placebo, and gallbladder inflammation (cholecystitis) in 0.6% versus 0.2%.
- Kidneys. Postmarketing reports describe acute kidney injury and worsening chronic renal failure, some requiring haemodialysis, often linked to dehydration from vomiting or diarrhoea.
- Eyes. In Ozempic’s own trial, diabetic retinopathy complications occurred more often on the drug (3.0%) than on placebo (1.8%). People with existing retinopathy should be monitored for progression.
- Allergic reactions. Serious hypersensitivity reactions, including anaphylaxis and angioedema, have been reported with Wegovy.
Ozempic and Wegovy are injections (Rybelsus is a tablet); see injection-site reactions for what is ordinary and what needs attention. Get emergency care for facial or throat swelling, difficulty breathing, or severe abdominal pain, and see what to do about a missed dose or overdose. This list is what the label discloses so a prescriber can watch for it, not a tool for self-diagnosis.
Why the grey market is a different problem than price
FDA has documented these problems in the unapproved semaglutide and tirzepatide market:
- Dosing errors, by patients giving themselves the wrong dose and by providers miscalculating it
- Counterfeit product, including fake Ozempic
- Unapproved salt forms (semaglutide sodium, semaglutide acetate) used in place of the approved active ingredient, which FDA says it knows of no lawful basis for using in compounding
- Fraudulent labels naming compounding pharmacies that don’t exist, or real pharmacies that didn’t make the product
- Hundreds of adverse event reports tied to compounded semaglutide, which FDA says are likely undercounted because pharmacies that aren’t outsourcing facilities don’t have to report them
FDA also publishes red flags for buying a GLP-1 online:
- Claims that a compounded drug is “the same” as the approved one
- Prices that look too good to be true
- Product that looks different from last time, or arrives damaged
- No screening by a licensed doctor
- Spelling errors or wrong pharmacy addresses on the label
Those problems are a reason to get semaglutide through the channel its approval covers.

Common questions
Is semaglutide legal?
Yes, as a prescription medicine sold as Ozempic, Wegovy and Rybelsus; it is not available over the counter. Compounded, counterfeit or "research use only" semaglutide sold directly to consumers is a different, unapproved supply chain. FDA has documented dosing errors, counterfeit product and fraudulent labelling in it.
What is semaglutide approved for?
Ozempic and oral Rybelsus are approved for blood-sugar control in type 2 diabetes. Ozempic is also approved to cut the risk of major cardiovascular events, and of kidney decline and cardiovascular death in chronic kidney disease, in adults with type 2 diabetes. Wegovy is approved for chronic weight management in adults and adolescents with obesity, or overweight plus a weight-related condition, and to cut the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight.
What is the boxed warning on semaglutide?
Semaglutide causes thyroid C-cell tumours in rodents, more with higher doses and longer treatment, at clinically relevant exposures. Whether this happens in humans is unknown. It must not be used by anyone with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.
How is semaglutide dosed?
The injection starts at 0.25 mg once a week and steps up every 4 weeks, to 0.5 mg, 1 mg and 1.7 mg. For weight management the maintenance dose is 2.4 mg once a week (or 1.7 mg). A prescriber manages this; it is not a number to copy from a forum.
Is compounded or "research" semaglutide the same product?
No. FDA has documented dosing errors, counterfeit product, fraudulent labelling and unapproved salt forms in the compounded and grey-market semaglutide supply. A vial labelled "research use only" and sold with dosing instructions is, in FDA's view, being sold for human use outside any approval.
Can I take semaglutide with another GLP-1 medicine or a research peptide?
The label says using it with any other GLP-1 receptor agonist, or another semaglutide product, is not recommended. Ask the prescriber managing your care about combining it with anything else.
Is Ozempic approved for weight loss?
No. Ozempic is approved only for adults with type 2 diabetes: blood-sugar control and cutting cardiovascular and kidney risk. Semaglutide's weight-management approval is under the brand Wegovy. Prescribing Ozempic for weight loss is legal off-label use, but outside what Ozempic's own label supports with trial data.
What are the side effects of semaglutide?
Gut effects are by far the most common. On Wegovy, 44% had nausea against 16% on placebo, 30% diarrhoea, 24% vomiting and 24% constipation. The boxed warning covers thyroid C-cell tumours seen in rodents. Less common but serious risks on the label include acute pancreatitis, gallbladder disease, acute kidney injury (postmarketing reports) and worsening diabetic retinopathy in people who already have it.
What should I avoid while using semaglutide?
Don't combine it with another GLP-1 receptor agonist or semaglutide product; the label says this is not recommended. The label also advises against it in severe gastroparesis, and warns that insulin or a sulfonylurea raises the risk of low blood sugar. Never share a pen, even with a new needle, because of infection risk.
How long do you stay on semaglutide for weight management?
Wegovy's label describes long-term treatment, to lose weight and keep it off, and its trials followed patients through 68 weeks. It is ongoing management of a chronic condition, not a short course. A prescriber sets the duration for each person.
Can I lose 20 pounds in a month with semaglutide?
No trial data supports that pace. In Wegovy's 68-week trials, weight loss built up gradually to an average of 9.6% to 16.0% of body weight by week 68, against 2.4% to 5.7% on placebo. Individual results vary, and a prescriber can set realistic expectations for you.
Does semaglutide get rid of belly fat specifically?
No. The trials behind its approval measured total body weight, not fat in any one area. Semaglutide works through appetite and slower stomach emptying, and no medicine can choose where the body loses fat first.
What's the cheapest way to get semaglutide?
As of 3 October 2026, the lowest Novo Nordisk price is $149 a month for the 1.5 mg Ozempic or Wegovy pill, a starting dose used for the first 30 days. At maintenance doses the pills ($199 to $299) still cost less than the pens ($349 to $499). With commercial insurance that covers it, Novo's savings offers cut the cost to as little as $25 a month. Eligible Medicare Part D members pay $50 a month for Wegovy through the GLP-1 Bridge.
How do I get semaglutide for $25 a month?
You need a prescription and commercial insurance that covers Ozempic or Wegovy. Novo Nordisk's savings offer then lowers the cost to as little as $25 a month, with at most $100 off each month, so a copay above $125 leaves you paying the copay minus $100. Government beneficiaries, including people on Medicare or Medicaid, are excluded.
How much is a 30 day supply of semaglutide?
At Novo Nordisk's self-pay prices on 3 October 2026, a month of Ozempic costs $349 to $499 for the pen or $149 to $299 for the pill. A month of Wegovy costs $349 to $399 for the pen or $149 to $299 for the pill. New patients pay $199 for their first 2 months of the lowest pen doses. List prices are $1,027.51 a month for Ozempic and $1,349.02 for Wegovy.
How much does a semaglutide injection cost?
At Novo Nordisk's self-pay prices on 3 October 2026, a month of the Ozempic pen costs $349 (0.25 to 1 mg) or $499 (2 mg). A month of the Wegovy pen costs $349 (0.25 to 2.4 mg) or $399 (Wegovy HD 7.2 mg). New patients pay $199 a month for their first 2 months of either pen at 0.25 or 0.5 mg, through 31 December 2026. List prices are $1,027.51 a month for Ozempic and $1,349.02 for Wegovy.
How much is semaglutide without insurance at Walmart?
I found no Walmart pharmacy page that prints a semaglutide price. Sam's Club, which Walmart owns, said on 7 January 2026 that its pharmacies would dispense the 1.5 mg Wegovy pill for $149 a month through Novo Nordisk's self-pay offer. TrumpRx.gov lists Novo's self-pay prices as a coupon, which it says retail pharmacies widely accept: $149 to $299 a month for the pills and $199 to $499 for the pens, as of 3 October 2026.
How much does a 3-month supply of Ozempic cost?
At list price, three pens cost $3,082.53. At Novo Nordisk's self-pay prices on 3 October 2026, three months of the 0.25 to 1 mg pen cost $1,047, and three months of 2 mg cost $1,497. A new patient starting on 0.25 mg pays $199, $199 and then $349, which is $747. With commercial insurance that covers Ozempic, Novo says you may pay as little as $25 for a prescription of up to 3 months, with savings capped at $100 a month.
Who cannot take Ozempic?
The label lists two contraindications: a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, and a past serious allergic reaction to semaglutide or any ingredient. It is approved only for adults with type 2 diabetes. It is not recommended in severe gastroparesis, and people with diabetic retinopathy should be monitored.
What organ is Ozempic hard on?
The label doesn't single out one organ. Its less common serious risks include acute pancreatitis, gallbladder disease such as gallstones, acute kidney injury (usually from dehydration after vomiting or diarrhoea) and worsening diabetic retinopathy. The boxed warning covers thyroid C-cell tumours seen in rodents; whether that happens in humans is unknown.
How long will it take to lose 20 lbs on Ozempic?
No trial measured time to a fixed number of pounds, and Ozempic is approved for type 2 diabetes, not weight loss; the weight-management brand is Wegovy. In Wegovy's 68-week trials, weight loss built up gradually to an average of 9.6% to 16.0% of body weight. How long 20 lb takes depends on starting weight and individual response.
What is the downside of semaglutide?
Nausea, diarrhoea, vomiting and constipation are common, especially while the dose is rising. A boxed warning covers thyroid C-cell tumours seen in rodents, and rarer serious risks include pancreatitis, gallbladder disease and kidney injury. It is also a long-term prescription treatment, and grey-market versions have FDA-documented quality problems.
What is the brand name of semaglutide?
Semaglutide is sold under the brand names Ozempic, Wegovy and Rybelsus, made by Novo Nordisk. Ozempic is approved for type 2 diabetes in adults: improving blood-sugar control alongside diet and exercise, and lowering the risk of major cardiovascular events (heart attack, stroke, cardiovascular death); the injection is also approved to lower the risk of worsening kidney function, kidney failure and cardiovascular death in adults with type 2 diabetes and chronic kidney disease. Wegovy is approved for injection: chronic weight management in adults with obesity or overweight with at least one weight-related condition and in children 12 years and older with obesity, lowering the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight, and noncirrhotic MASH (fatty-liver inflammation) with moderate to advanced liver fibrosis in adults (accelerated approval); tablets: the same weight-management and cardiovascular uses in adults only. Rybelsus is approved for type 2 diabetes in adults: improving blood-sugar control alongside diet and exercise, and lowering the risk of major cardiovascular events in those at high risk.
What is the generic name for Ozempic?
The generic name for Ozempic is semaglutide. What is the generic name for Wegovy?
The generic name for Wegovy is semaglutide. Wegovy is Novo Nordisk's brand, approved for injection: chronic weight management in adults with obesity or overweight with at least one weight-related condition and in children 12 years and older with obesity, lowering the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight, and noncirrhotic MASH (fatty-liver inflammation) with moderate to advanced liver fibrosis in adults (accelerated approval); tablets: the same weight-management and cardiovascular uses in adults only.
What is the generic name for Rybelsus?
The generic name for Rybelsus is semaglutide. Rybelsus is Novo Nordisk's brand, approved for type 2 diabetes in adults: improving blood-sugar control alongside diet and exercise, and lowering the risk of major cardiovascular events in those at high risk.
Is Ozempic the same as semaglutide?
Yes. Ozempic is a brand of semaglutide, made by Novo Nordisk: the active ingredient is semaglutide. It comes as a once-weekly injection (multi-dose pen or single-dose prefilled syringe); also once-daily Ozempic tablets, dosed at injection: 0.25 mg once weekly to start, up to 2 mg once weekly; tablets: 1.5 mg once daily to start, up to 9 mg once daily. Compounded or research-vial semaglutide is not Ozempic and has not been through its FDA review.
Is Wegovy the same as semaglutide?
