Survodutide Dosage: Common Protocol, Trial Doses and Calculator

Survodutide dosage: the common protocol (0.6-6.0 mg weekly) plus the doses its published Phase 2 and Phase 3 trials used - not FDA-approved.

The short version

Survodutide has no approved dose because it has no approved product. The common protocol is a once-weekly subcutaneous injection starting around 0.6 mg. Sources differ on the maintenance ceiling. A phase 2 dosing-guide source titrates over about 20 weeks to 2.4-4.8 mg. The published phase 3 SYNCHRONIZE-1 trial uses a slower, flexible up-titration to a higher 3.6 or 6.0 mg target. This is common practice, not a trial-established or FDA-approved dose. Two published Phase 2 trials tested it directly, up to 4.8 mg (obesity) and 6.0 mg (MASH).

  • The obesity trial tested 0.6, 2.4, 3.6 and 4.8 mg once weekly, by injection, for 46 weeks.
  • The separate MASH (liver disease) trial tested 2.4, 4.8 and 6.0 mg once weekly, for 48 weeks.
  • No FDA or other regulator has reviewed or approved any survodutide dose for any use.
  • Work out reconstitution and volume with a calculator, not a forum post.

Survodutide has no approved dose, because it has no approved product. This page reports what two separate, peer-reviewed human trials used. It is not a recommendation for what anyone should take. For what survodutide is, its mechanism, and its wider trial programme, see the main survodutide guide.

US legal status - Research use only

Survodutide has no FDA-approved product and no approved indication or dose. It remains an investigational drug inside sponsor-run trials; outside those trials it is sold as an unapproved "research use only" peptide.

Survodutide dosage: the common protocol

The protocol most often used is a once-weekly subcutaneous injection starting around 0.6 mg. Sources differ on the maintenance ceiling. A phase 2 dosing-guide source describes titrating over about 20 weeks to a 2.4-4.8 mg maintenance dose. The published phase 3 SYNCHRONIZE-1 trial uses a slower, flexible up-titration, reassessed roughly every 2-4 weeks, to a higher 3.6 or 6.0 mg maintenance target. This is common practice, not a single trial-established or FDA-approved dose.

Upgraded 2026-09-27: added the published phase 3 SYNCHRONIZE-1 trial (Roux et al., Diabetes, Obesity and Metabolism, via PMC), which materially raises the maintenance ceiling - participants are randomized to a 3.6 mg or 6.0 mg weekly maintenance dose (vs. the phase 2 trial's 4.8 mg maximum), reached via a slower, flexible up-titration with dose increases roughly every 2-4 weeks and allowance for re-escalation attempts if GI side effects occur, over a 76-week treatment period. The phase 3 paper doesn't state the exact starting dose or step-by-step ladder, so those figures are kept from the re-verified phase 2 dosing-guide source (0.6 mg start, arms of 0.6/2.4/3.6/4.8 mg, 20-week escalation then 26 weeks at steady dose). Range now spans both trials' maintenance doses; agreement is 'varies' because phase 2 and phase 3 target different maintenance ceilings. Dropped the second, redundant community dosing-guide source since the phase 3 publication supersedes it.

Survodutide doses used in human studies: obesity trial

A Phase 2, randomised, double-blind, placebo-controlled, dose-finding trial le 2024 - le Roux CW, Steen O, Lucas KJ et al. Glucagon and GLP-1 receptor dual agonist survodutide for obesity: a randomised, double-blind, placebo-controlled, dose-finding phase 2 trial. Lancet Diabetes Endocrinol. 2024;12(3):162-173., in the references below enrolled 387 adults aged 18-75 without diabetes, with a BMI of 27 kg/m² or higher. It ran at 43 centres in 12 countries. Participants were randomly assigned to one of four survodutide doses or placebo:

Weekly doseRouteSchedule
0.6 mgSubcutaneous injectionOnce weekly, 46 weeks total (20 weeks dose escalation, 26 weeks maintenance)
2.4 mgSubcutaneous injectionSame schedule
3.6 mgSubcutaneous injectionSame schedule
4.8 mgSubcutaneous injectionSame schedule
PlaceboSubcutaneous injectionSame schedule

The paper reports mean bodyweight change from baseline to week 46, by planned dose: -6.2% (0.6 mg), -12.5% (2.4 mg), -13.2% (3.6 mg) and -14.9% (4.8 mg). Placebo gave -2.8%. The paper describes this dose-dependent pattern as its main efficacy finding. Adverse events, mostly gastrointestinal, occurred in 91% of survodutide recipients pooled across doses versus 75% on placebo.

Survodutide doses used in human studies: MASH (liver disease) trial

A separate Phase 2 trial Sanyal 2024 - Sanyal AJ, Bedossa P, Fraessdorf M et al. A Phase 2 Randomized Trial of Survodutide in MASH and Fibrosis. N Engl J Med. 2024;391(4):311-319., in the references below enrolled adults with biopsy-confirmed MASH (metabolic dysfunction-associated steatohepatitis) and liver fibrosis stage F1 through F3. They were randomly assigned 1:1:1:1 to one of three survodutide doses or placebo:

Weekly doseRouteSchedule
2.4 mgSubcutaneous injectionOnce weekly, 48 weeks total (24 weeks rapid dose escalation, 24 weeks maintenance)
4.8 mgSubcutaneous injectionSame schedule
6.0 mgSubcutaneous injectionSame schedule
PlaceboSubcutaneous injectionSame schedule

The paper reports histologic improvement in MASH with no worsening of fibrosis in 47% (2.4 mg), 62% (4.8 mg) and 43% (6.0 mg) of participants. On placebo it was 14%. Survodutide caused more nausea (66% vs 23%), diarrhoea (49% vs 23%) and vomiting (41% vs 4%) than placebo. Serious adverse events occurred in 8% of survodutide recipients and 7% on placebo.

How to reconstitute survodutide

The injection volume for a target dose depends on how a specific vial was reconstituted: how much diluent, and what resulting concentration. This page cannot know that for your vial. The dosage calculator walks through vial strength, diluent volume and target amount. It does the math; it does not tell you what dose to use.

What is not established

  • No approved dose exists, for any population, indication or duration, because no survodutide product has completed FDA review.
  • No dose has been proven safe or effective by any regulator. The figures above are what two trials measured in their own participants, not a demonstrated general result. No regulatory decision has followed either trial.
  • No maintenance dose beyond either trial’s follow-up window is established. Both trials measured outcomes through their own end dates (46 and 48 weeks); nothing published tracks a dose beyond that.
  • The common protocol above is not the trial protocol. It reflects what dosing-guide sources describe as typical practice, not a dose any trial, paper or regulator has established as safe or effective.

Who should not use survodutide

No FDA-reviewed contraindication list exists for survodutide, because no such review has happened. The related FDA-approved GLP-1 drugs semaglutide and tirzepatide are contraindicated in anyone with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. Neither is established as safe in pregnancy or breastfeeding.

Survodutide’s own published trials excluded participants with diabetes (obesity trial) and did not establish safety in pregnancy, breastfeeding or children in either trial. An unapproved compound with no FDA-reviewed label should not be self-directed around any medical condition, current medication or pregnancy status.

Survodutide side effects and risks

Both published trials report gastrointestinal adverse events (nausea, diarrhoea, vomiting) as the most common. They occurred more often on survodutide than on placebo. The obesity trial reports these in 75% of survodutide recipients pooled across doses, versus 42% on placebo. The MASH trial reports serious adverse events in 8% of survodutide recipients versus 7% on placebo.

There is no long-term safety data past the trials’ own follow-up windows. A vial bought outside the trial system carries no verified content, purity or strength.

Common questions

What is the usual survodutide dose?

The common protocol is a once-weekly subcutaneous injection starting around 0.6 mg. Sources differ on the maintenance ceiling. A phase 2 dosing-guide source titrates over about 20 weeks to 2.4-4.8 mg. The published phase 3 SYNCHRONIZE-1 trial uses a slower, flexible up-titration to a higher 3.6 or 6.0 mg target. This is common practice reported outside the trial system. No regulator has approved a dose or shown one to be safe or effective outside a study.

What dose of survodutide did the human trials use?

Two separate published Phase 2 trials. In adults with overweight or obesity without diabetes, it was tested at 0.6, 2.4, 3.6 and 4.8 mg once weekly. In adults with biopsy-confirmed MASH and liver fibrosis, it was tested at 2.4, 4.8 and 6.0 mg once weekly. These are trial-assigned doses, not a recommended dose for anyone outside those trials.

Is there an FDA-approved survodutide dose?

No. Survodutide has no FDA-approved product and no approved dose for any use. The figures on this page come from two published Phase 2 trials, not from FDA review or an approved label.

How often and for how long was survodutide dosed in the trials?

Once weekly by subcutaneous injection in both published trials. The obesity trial ran 46 weeks (20 weeks of dose escalation, then 26 weeks at a maintained dose). The MASH trial ran 48 weeks (24 weeks of escalation, then 24 weeks of maintenance). Trial staff set and monitored both schedules.

How do I work out reconstitution and injection volume?

That is arithmetic. It depends on the concentration of your specific vial and diluent, which this page cannot know. Use the dosage calculator to work through the math for your vial and target amount. It does not tell you what dose to use.

What are the main risks?

Both published trials report gastrointestinal adverse events (nausea, diarrhoea, vomiting) as the most common. They occurred more often on survodutide than on placebo and rose with dose. The MASH trial also reports serious adverse events in 8% of survodutide recipients versus 7% on placebo. There is no long-term safety data past the trials' own follow-up windows. A vial bought outside the trial system has no verified content, purity or strength.

When should I stop and get medical care?

Get emergency care (call 911 in the US) for a severe allergic reaction, severe or persistent vomiting with signs of dehydration, or severe abdominal pain. Abdominal pain can signal pancreatitis. Call Poison Control at 1-800-222-1222 if you have taken an unknown or excessive amount of anything sold as survodutide. This page is not a substitute for a clinician who knows your history.
