Oral Semaglutide: Wegovy Pill, Ozempic Pill and Rybelsus Compared

Oral semaglutide pills: Wegovy for weight, Ozempic pill and Rybelsus for diabetes. $149 to $299 a month self-pay on 3 October 2026. Doses, trials, side effects.

The short version

Oral semaglutide is semaglutide, the drug in the Ozempic and Wegovy injections, made as a once-daily tablet. Three prescription versions are sold in the US: the Wegovy pill for weight management, and Rybelsus and its replacement, the Ozempic pill, for type 2 diabetes. Each tablet contains an absorption enhancer called SNAC, so it is swallowed on an empty stomach with a little water, 30 minutes before food. As of 3 October 2026, Novo Nordisk sells the pills for $149 to $299 a month without insurance. Compounded and research-labelled versions are not FDA-approved.

  • The FDA-approved semaglutide tablets are the Wegovy pill for weight, and the Ozempic pill and Rybelsus for type 2 diabetes.
  • Self-pay prices on 3 October 2026 are $149 to $299 a month for either pill; eligible Medicare members pay $50 for the Wegovy pill through the GLP-1 Bridge.
  • Only 1% to 2% of the semaglutide in the current tablets is absorbed. Even so, the 25 mg daily Wegovy pill is predicted to give blood levels like the 2.4 mg weekly injection.
  • In its 64-week trial, the 25 mg Wegovy pill reduced body weight by 13.6%, against 2.4% on placebo.
  • Compounded sublingual and dissolving semaglutide tablets are not FDA-approved, and I found no human trial of them.

Oral semaglutide is semaglutide taken as a once-daily tablet instead of a weekly injection. In the US it is sold by prescription as the Wegovy pill for weight management, and as Rybelsus and its replacement, the Ozempic pill, for type 2 diabetes. As of 3 October 2026, Novo Nordisk’s self-pay price for the pills is $149 to $299 a month.

“Semaglutide pills”, “semaglutide tablets” and “oral semaglutide” usually mean these three products. Anything else sold under those names is compounded or research-labelled, and none of it is FDA-approved.

The approved semaglutide pills compared

All three are made by Novo Nordisk and taken once a day. Prices are Novo’s self-pay prices for a bottle of 30 tablets, checked on 3 October 2026.

RybelsusThe same as the Ozempic pill3 mg for 30 days, then 7 mg; 14 mg if more control is neededThe sameNo cash price found; being replaced by the Ozempic pill

The FDA approved Rybelsus on 20 September 2019, as the first semaglutide tablet. Novo announced the Wegovy pill’s approval on 22 December 2025, and expected to launch it in early January 2026. NovoCare now says Rybelsus “is being replaced by Ozempic pill”, and switching needs a new prescription.

Rybelsus and the Ozempic pill share one label. It states that the two are not substitutable milligram for milligram: NovoCare matches Ozempic tablets of 1.5, 4 and 9 mg to Rybelsus 3, 7 and 14 mg.

Another GLP-1 pill, Foundayo, contains a different drug: orforglipron, made by Eli Lilly, which is not a peptide. Its label lets it be taken with or without food.

How a semaglutide pill gets absorbed

Peptides such as semaglutide are usually broken down in the gut and barely absorbed (Buckley 2018 - Buckley ST, Bækdal TA, Vegge A et al. Transcellular stomach absorption of a derivatized glucagon-like peptide-1 receptor agonist. Sci Transl Med. 2018;10(467)., in the references below). The tablets get around this with an absorption enhancer, salcaprozate sodium (SNAC).

In human and dog studies, semaglutide from the tablet was absorbed in the stomach, close to the tablet’s surface, and only when SNAC was in the formulation (Buckley 2018 - Buckley ST, Bækdal TA, Vegge A et al. Transcellular stomach absorption of a derivatized glucagon-like peptide-1 receptor agonist. Sci Transl Med. 2018;10(467)., in the references below). SNAC buffers the area around the tablet, which protects semaglutide from digestive enzymes, and its effect on absorption is brief.

Even so, very little gets through. The labels estimate absolute bioavailability at:

  • 0.4% to 1% for Rybelsus 3, 7 and 14 mg;
  • 1% to 2% for Ozempic tablets of 1.5, 4 and 9 mg;
  • 1% to 2% for the Wegovy tablets.

That is why the oral doses are so much larger than the injected ones. The newer tablets (Ozempic pill and Wegovy pill) absorb better than Rybelsus. In healthy volunteers, Ozempic tablets of 1.5, 4 and 9 mg gave blood levels like Rybelsus 3, 7 and 14 mg.

Why the fasting rules matter

The label’s studies found more semaglutide was absorbed with 50 mL of water than with 240 mL, and no meaningful difference between 50 and 120 mL (about 4 ounces). It was also absorbed better the longer people waited before eating, across waits of 15, 30, 60 and 120 minutes. So the labels set these rules:

  • one tablet each morning, on an empty stomach;
  • plain water only, up to 4 ounces;
  • swallowed whole, not split, crushed, chewed or dissolved;
  • at least 30 minutes before any food, drink or other oral medicine;
  • a missed tablet is skipped, and the next one taken the following day.

Pill versus injection blood levels

In people with obesity or overweight but no diabetes, the Wegovy label predicts the 25 mg tablet gives average semaglutide levels similar to the 2.4 mg weekly injection. Levels vary more between people on the tablet: 90% fell between 27 and 186 nmol/L, against 51 and 110 nmol/L on the injection.

In people with type 2 diabetes, the tablet gave lower average levels than the injection (44 against 68 nmol/L). The label warns some of them may get too little semaglutide, and suggests switching to the injection if the response is inadequate.

Doses compared with the injection

Oral and injected semaglutide are not compared milligram for milligram. These are the switches the labels allow:

FromToWhen
Wegovy 2.4 mg weekly injectionWegovy 25 mg tablet dailyOne week after the last injection
Wegovy 25 mg tabletWegovy 2.4 mg weekly injection (1.7 mg if 25 mg was not tolerated)The day after the last tablet
Ozempic 0.5 mg weekly injectionRybelsus 7 or 14 mg, or Ozempic tablets 4 or 9 mgOne week after the last injection
Rybelsus 7 mg or 14 mg, or Ozempic tablets 4 mg or 9 mgThe matching dose of the other tablet (7 mg to 4 mg, 14 mg to 9 mg, or back)The day after the last tablet, and not in the first 30 days

The injections reach higher doses than the tablets. The Wegovy pen goes up to 7.2 mg a week for weight loss, and the Ozempic pen up to 2 mg. Full injection schedules: semaglutide dosage.

Wegovy pill schedule for weight loss or cardiovascular risk in adults:

DaysDose, once daily
1-301.5 mg
31-604 mg
61-909 mg
91 onward25 mg

The label allows a slower build-up if a dose is not tolerated. If 25 mg is not tolerated, it suggests switching to the 1.7 mg weekly injection.

Ozempic pill and Rybelsus schedule for type 2 diabetes: 1.5 mg (Ozempic) or 3 mg (Rybelsus) for the first 30 days, which the label says does not control blood sugar. Then 4 mg or 7 mg daily, rising to 9 mg or 14 mg from day 61 if more control is needed. The dose is always the prescriber’s decision.

What the trials showed

Novo Nordisk funded PIONEER 6, SOUL, OASIS 1 and OASIS 4.

I found no head-to-head trial of the Wegovy pill against the injection. A Novo-funded indirect comparison of OASIS 4 and the injection’s STEP 1 trial found similar weight loss on the 25 mg tablet and the 2.4 mg injection (Plotkin 2026 - Plotkin M, Ivkovic M, Smith I et al. Semaglutide 25 mg Oral Versus Semaglutide 2.4 mg Injectable: An Indirect Treatment Comparison of Weight Loss Outcomes. Diabetes Obes Metab. 2026;28(8):7237-7246., in the references below). The authors included Novo Nordisk employees and advisers.

Side effects

Rates from the placebo-controlled trials of semaglutide tablets in adults with type 2 diabetes, per the Rybelsus and Ozempic tablets label:

ReactionPlacebo7 mg14 mg
Nausea6%11%20%
Abdominal pain4%10%11%
Diarrhoea4%9%10%
Decreased appetite1%6%9%
Vomiting3%6%8%
Constipation2%6%5%

Most nausea, vomiting and diarrhoea happened while the dose was going up. Gut side effects led 8% on 14 mg and 4% on 7 mg to stop, against 1% on placebo.

For the 25 mg Wegovy pill, the label says common side effects were similar in type and frequency to the injection’s. In its trial:

  • 6.9% stopped because of side effects, against 5.9% on placebo;
  • severe gut side effects affected 2%, against 0%;
  • gallstones affected 2.5%, against 1%;
  • altered skin sensation (dysaesthesia) affected 5%, against none on placebo.

Some warnings come from the tablet itself. SNAC or its breakdown products pass into breast milk, so breastfeeding is not recommended on the tablets; semaglutide forms without SNAC exist. In a study with the semaglutide tablet, levothyroxine exposure rose 33%, and the label advises monitoring oral medicines taken alongside it.

Who should not take it: anyone with a personal or family history of medullary thyroid carcinoma or MEN 2. The same applies after a serious allergic reaction to semaglutide or an ingredient in the tablet. The labels also:

  • do not recommend the tablets in severe gastroparesis;
  • warn of pancreatitis, gallbladder disease, and kidney injury after dehydration;
  • warn that low blood sugar is more likely alongside insulin or a sulfonylurea;
  • note a risk of inhaling stomach contents under anaesthesia or deep sedation;
  • say to stop the tablets at least 2 months before a planned pregnancy.

How much oral semaglutide costs

These prices come from Novo Nordisk, TrumpRx.gov and Costco Pharmacy, checked on 3 October 2026. All three showed the same self-pay prices. One bottle of 30 tablets is a month.

Pill and doseSelf-pay price a monthList price
Wegovy pill 1.5 mg$149$1,349.02 a package
Wegovy pill 4 mg$199$1,349.02
Wegovy pill 9 mg or 25 mg$299$1,349.02
Ozempic pill 1.5 mg$149$1,027.51 a package
Ozempic pill 4 mg$199$1,027.51
Ozempic pill 9 mg$299$1,027.51
  • With commercial insurance that covers the pill, Novo’s savings offers bring the cost to as little as $25 a month, with at most $100 off a month. NovoCare says the Ozempic pill has the same coverage as Rybelsus.
  • With Medicare, the GLP-1 Bridge covers the Wegovy tablet for weight management at $50 a month, from 1 July 2026 to 31 December 2027. It excludes people with type 2 diabetes and does not cover the Ozempic pill or Rybelsus.
  • In 2027, Novo says the list price of Wegovy 25 mg tablets and Rybelsus 7 mg and 14 mg will fall to $675 from 1 January. It says self-pay prices are not affected.

At self-pay prices the pills cost less than the pens, which run $349 to $499 a month after new-patient offers. The legal routes to semaglutide online, with prices: semaglutide online.

Compounded “oral semaglutide”

No compounded semaglutide is FDA-approved. FDA says it does not review compounded drugs for safety, effectiveness or quality. It says they should only be used when a patient’s needs cannot be met by an approved drug.

Compounded “oral” semaglutide is usually not a swallowed tablet. A survey of compounding sites in early 2025 found 17 single-ingredient semaglutide or tirzepatide products that differed from the approved ones. Of these, 82% were sublingual (dissolved under the tongue) and 18% were orally disintegrating tablets (Belcourt 2026 - Belcourt J, Sapowadia A, White CM. Compounded Semaglutide and Tirzepatide Products Use Unique Formulations but Efficacy and Safety Largely Unknown. Ann Pharmacother. 2026;60(9):902-906., in the references below). The authors found it had not been shown whether these forms offer any advantage over the approved tablets.

What I found on how these products perform:

FDA has acted on how these products are sold. In a warning letter of 9 September 2025, it cited a company, Elevated, for calling its “Semaglutide ODT” a dissolvable tablet with “the same active ingredient” as leading GLP-1 medicines. FDA said compounded drugs are not FDA-approved, so such claims are false or misleading.

The legal room for copies has narrowed:

  • FDA declared the semaglutide injection shortage over on 21 February 2025. The grace periods for pharmacies and outsourcing facilities ended in 2025.
  • FDA treats a compounded drug as “essentially a copy” if it has the same active ingredient at a similar strength and the same route as an approved product. The exception is a prescriber documenting a significant difference for an identified patient. Approved semaglutide tablets are swallowed.
  • FDA says it knows of no lawful basis for compounding with the salt forms semaglutide sodium or semaglutide acetate.
  • On 30 April 2026, FDA proposed excluding semaglutide from the list outsourcing facilities can compound from.
  • Its notice reviewed requests to compound semaglutide as sublingual, buccal and oral products. FDA found no basis to conclude that the approved tablets and injections are medically unsuitable for certain patients, or that a sublingual or buccal product would solve such a problem. Comments closed on 30 July 2026, and I found no final decision as of 3 October 2026.

As of 31 May 2026, FDA had received 990 adverse-event reports linked to compounded semaglutide. More on the legal differences: compounded vs branded semaglutide.

Research-labelled semaglutide: not a pill, and not the approved medicine

These are research-use products, not the Wegovy pill, the Ozempic pill or Rybelsus. On 3 October 2026 I checked the full catalogues of the three active sellers in our vendor registry. None sells semaglutide as a tablet, capsule or drops. Two sell it in vials, an injectable format.

Peptide Decode has no commercial relationship with any of these sellers and earns nothing from these links.

Next Level Pharm also sells semaglutide blended with cagrilintide, as “Cagrisema” vials. Spartan includes its 3 mg vial in a three-vial “Skinny & Fit” stack. BioEdge Research Labs sells no semaglutide product.

How these differ from the approved pills:

FDA has acted against research-labelled semaglutide. It warned Summit Research Peptides (10 December 2024), Prime Sciences (31 March 2026) and Peak Performance Peptides (24 August 2026) that their semaglutide products were unapproved new drugs. Each seller labelled them for research only; FDA said the evidence showed they were intended for human use. FDA also urges consumers not to buy GLP-1s falsely labelled “for research purposes”. I found no warning letter to the three sellers in our registry.

This page gives no dosing for research vials. The dosing for approved semaglutide comes from its FDA label, above. Every route with dated prices, and the research-labelled vials in their own section: where to buy semaglutide.

Is oral semaglutide the same as Ozempic or Wegovy?

The molecule is the same: semaglutide is the only active ingredient in all of them. The products differ in form, dose, approval and routine, and their milligrams do not line up. The molecule across its brands: semaglutide guide. How the drug class works: how GLP-1 medicines work. No tirzepatide pill is approved: tirzepatide pills. Which other peptides work by mouth: oral peptides.

Common questions

Does oral semaglutide really work?

In its trials, yes. Per the Wegovy label, the 25 mg Wegovy pill reduced body weight by 13.6% over 64 weeks, against 2.4% on placebo. In type 2 diabetes, 14 mg of Rybelsus lowered HbA1c by 1.4 points over 26 weeks, against 0.3 on placebo. In a trial of 9,650 high-risk adults with type 2 diabetes, up to 14 mg lowered the risk of cardiovascular death, heart attack or stroke (hazard ratio 0.86). How much is absorbed depends on following the label's fasting routine.

How much does the semaglutide oral pill cost?

On 3 October 2026, Novo Nordisk's self-pay price for a 30-tablet bottle of the Wegovy pill was $149 (1.5 mg), $199 (4 mg) or $299 (9 mg or 25 mg). The Ozempic pill costs $149 (1.5 mg), $199 (4 mg) or $299 (9 mg). TrumpRx.gov and Costco Pharmacy showed the same prices. I found no published cash price for Rybelsus, which the Ozempic pill is replacing.

How much will the new Wegovy pills cost?

Novo Nordisk expected to launch the Wegovy pill in early January 2026. On 3 October 2026 it cost $149 to $299 a month self-pay, depending on the dose, with a list price of $1,349.02 a package. Novo Nordisk has announced the list price of the 25 mg tablet will fall to $675 from 1 January 2027, and says self-pay prices are not affected. Eligible Medicare Part D members pay $50 a month through the GLP-1 Bridge, which runs to 31 December 2027.

Will insurance pay for oral semaglutide?

It depends on your plan. With commercial insurance that covers the pill, Novo Nordisk's savings offers bring the cost to as little as $25 a month, with at most $100 off a month. NovoCare says the Ozempic pill has the same coverage as Rybelsus. Medicare's GLP-1 Bridge covers the Wegovy tablet for $50 a month, but excludes people with type 2 diabetes, whose Part D plan might cover a GLP-1 instead.

Can you buy oral semaglutide over the counter?

No. The Wegovy pill, the Ozempic pill and Rybelsus are all prescription-only, and compounded versions also need a prescription. Research-labelled semaglutide comes in vials, not tablets, and is labelled not for human use. Two of the three sellers I checked sell it, and neither sells tablets. FDA has sent warning letters to sellers of research-labelled semaglutide.

Is semaglutide better in pill form or injection?

Neither is better for everyone. In people without diabetes, the Wegovy label predicts the 25 mg tablet gives average blood levels similar to the 2.4 mg weekly injection, but more variable. In type 2 diabetes the tablet gives lower levels. The pill is daily with a fasting routine and needs no needles or refrigerator. The injection is weekly and goes up to 7.2 mg for weight loss. Breastfeeding is not recommended on the tablets.

What is the downside of Rybelsus?

The daily routine is strict: an empty stomach, up to 4 ounces of plain water, then 30 minutes before any food, drink or other pills. Nausea affected 20% on 14 mg against 6% on placebo, and 8% on 14 mg stopped because of gut side effects. Weight loss was modest in type 2 diabetes: 3.7 kg on 14 mg against 1.4 kg on placebo over 26 weeks. Novo Nordisk is also replacing it with the Ozempic pill.

Why is Rybelsus being discontinued?

Novo Nordisk says Rybelsus is being replaced by the Ozempic pill. NovoCare says Ozempic tablets of 1.5, 4 and 9 mg work similarly to Rybelsus 3, 7 and 14 mg, with the same efficacy and safety and the same insurance coverage. Switching needs a new prescription. NovoCare gives no reason beyond the replacement and no end date.

What are Willow semaglutide tablets?

Willow sells compounded medicines online. Its semaglutide tablets page, checked on 3 October 2026, describes compounded semaglutide tablets placed under the tongue rather than swallowed, at $299 a month. The page says compounded semaglutide tablets are not approved by the FDA. They are not the Wegovy pill, the Ozempic pill or Rybelsus.

How long does it take to lose 20 lbs on oral semaglutide?

I found no trial that measured the time to lose a fixed number of pounds. The Wegovy pill takes 90 days to reach its 25 mg maintenance dose, and weight came off gradually in its trial, with the 13.6% average loss measured at week 64. How fast one person loses depends on starting weight, dose and response.

Can I drink coffee with oral semaglutide?

Not with the tablet. The labels say to take it with plain water only, up to 4 ounces, and then wait at least 30 minutes before any food, drink or other oral medicine. After that wait, I found no restriction on coffee in the labels.

Is compounded oral semaglutide the same as Rybelsus or the Wegovy pill?

No. The approved tablets pair semaglutide with the absorption enhancer SNAC, and their absorption was measured in Novo Nordisk's own studies. Compounded tablets and drops are not FDA-approved or FDA-reviewed, and the single-ingredient ones a survey of compounding sites found were all sublingual or dissolving tablets. FDA has cited a company for claiming its dissolving semaglutide tablet delivers the same active ingredient as the leading GLP-1 medicines.

What are the side effects of semaglutide pills?

Mostly gut side effects. In the diabetes trials, nausea affected 20% on 14 mg against 6% on placebo. Abdominal pain, diarrhoea, decreased appetite, vomiting and constipation followed, mostly while the dose was going up. On the 25 mg Wegovy pill, 5% reported altered skin sensation and 2.5% gallstones. The labels carry a boxed warning about thyroid C-cell tumours seen in rodents.

Can I switch from the Wegovy injection to the pill?

The label allows it from the 2.4 mg dose. One week after the last 2.4 mg injection, the 25 mg tablet starts once daily. Going the other way, the 2.4 mg weekly injection starts the day after the last 25 mg tablet. The prescriber decides whether and when to switch.
