Best Peptides for Inflammation, Ranked by Human Evidence
Thymosin alpha-1 has the most human trials, but not of inflammation itself. KPV, BPC-157 and TB-500 have little or no human data. Ranked by human evidence.
The short version
Thymosin alpha-1 has more human trials than any other peptide sold for inflammation. Those trials tested infection and critical-illness outcomes, and the largest found no clear survival benefit. ARA-290 and LL-37 have a few small trials. KPV, BPC-157 and TB-500, the peptides most often sold for inflammation, rest almost entirely on animal and cell studies. None is FDA-approved.
- Thymosin alpha-1 has 64 PubMed records tagged as randomised trials, mostly in sepsis and hepatitis B.
- An LL-37 cream helped foot ulcers heal but did not lower the inflammatory markers IL-1α and TNF-α.
- BPC-157's one placebo-controlled trial, in ulcerative colitis, found no significant difference from placebo.
- FDA found no human exposure data for KPV or TB-500 by any route.
Thymosin alpha-1 has the most human trials of any peptide sold for inflammation. They measured infection and survival outcomes, and the largest found no clear benefit. KPV, BPC-157 and TB-500, the peptides sold most often for inflammation, have little or no human data.
Inflammation is not one condition, and few of the trials below measured it directly. The ranking orders each peptide by the strongest human evidence of any kind and says what each trial actually measured. A higher rank means more and better trials, not that a peptide calms inflammation.

1. Thymosin alpha-1: the most trials, but not of inflammation
Multiple randomised trials, though with limitations. 64 PubMed records tagged as randomised trials, including multicentre trials in sepsis and pancreatitis. The largest found no clear survival benefit, and the trials reviewed here tested infection and critical illness, not chronic inflammation.
Thymosin alpha-1 is a 28-amino-acid immune peptide. Trials have used it to boost the immune response in infection and critical illness. It has been tested as an immune enhancer, not an anti-inflammatory.
The largest trial randomised 1,106 adults with sepsis in 22 Chinese hospitals to thymosin alpha-1 or placebo injections for seven days (Wu 2025 - Wu J, Pei F, Zhou L et al. The efficacy and safety of thymosin α1 for sepsis (TESTS): multicentre, double blinded, randomised, placebo controlled, phase 3 trial. BMJ. 2025;388:e082583., in the references below). Deaths at 28 days were 23.4% against 24.1%, with no clear difference. SciClone Pharmaceuticals, which sells it as Zadaxin, helped fund the trial.
The smaller trials point in different directions:
- Severe sepsis, 361 patients. Deaths were 26.0% against 35.0%, a borderline result in a single-blind trial (Wu 2013 - Wu J, Zhou L, Liu J et al. The efficacy of thymosin alpha 1 for severe sepsis (ETASS): a multicenter, single-blind, randomized and controlled trial. Crit Care. 2013;17(1):R8., in the references below).
- Severe acute pancreatitis, 508 patients. It did not reduce infection of dead pancreatic tissue (Ke 2022 - Ke L, Zhou J, Mao W et al. Immune enhancement in patients with predicted severe acute necrotising pancreatitis: a multicentre double-blind randomised controlled trial. Intensive Care Med. 2022;48(7):899-909., in the references below).
- Chronic hepatitis B, 62 patients. Compared with interferon, it gave similar or better viral responses after follow-up, and it was better tolerated (You 2006 - You J, Zhuang L, Cheng HY et al. Efficacy of thymosin alpha-1 and interferon alpha in treatment of chronic viral hepatitis B: a randomized controlled study. World J Gastroenterol. 2006;12(41):6715-21., in the references below).
Thymosin alpha-1 is not FDA-approved. FDA’s December 2024 review says it is not approved in the US, Japan or Europe, except Italy. In the US it is sold online as a research chemical.
2. ARA-290 (cibinetide): small trials in an inflammatory disease
Multiple randomised trials, though with limitations. Four small placebo-controlled trials, each lasting about four weeks, in sarcoidosis and type 2 diabetes. They measured nerve symptoms and nerve fibres, not inflammation itself.
ARA-290 is a peptide built from part of erythropoietin and designed to switch on a tissue-repair receptor that counters inflammation (Dahan 2013 - Dahan A, Dunne A, Swartjes M et al. ARA 290 improves symptoms in patients with sarcoidosis-associated small nerve fiber loss and increases corneal nerve fiber density. Mol Med. 2013;19(1):334-45., in the references below). Its trials are in sarcoidosis, an inflammatory disease, and in type 2 diabetes. They measured nerve damage rather than inflammation.
- Sarcoidosis, 22 people. Neuropathy symptom scores improved more on ARA-290 than placebo over four weeks. Pain and fatigue improved equally in both groups (Heij 2012 - Heij L, Niesters M, Swartjes M et al. Safety and efficacy of ARA 290 in sarcoidosis patients with symptoms of small fiber neuropathy: a randomized, double-blind pilot study. Mol Med. 2012;18(1):1430-6., in the references below).
- Sarcoidosis, 64 people. Only the 4 mg dose significantly increased corneal nerve fibres. Pain improved in every group, including placebo (Culver 2017 - Culver DA, Dahan A, Bajorunas D et al. Cibinetide Improves Corneal Nerve Fiber Abundance in Patients With Sarcoidosis-Associated Small Nerve Fiber Loss and Neuropathic Pain. Invest Ophthalmol Vis Sci. 2017;58(6):BIO52-BIO60., in the references below).
- Type 2 diabetes. Blood sugar control and nerve symptoms improved over 28 days of daily injections (Brines 2015 - Brines M, Dunne AN, van Velzen M et al. ARA 290, a nonerythropoietic peptide engineered from erythropoietin, improves metabolic control and neuropathic symptoms in patients with type 2 diabetes. Mol Med. 2015;20(1):658-66., in the references below). A second sarcoidosis trial reported similar nerve findings (Dahan 2013 - Dahan A, Dunne A, Swartjes M et al. ARA 290 improves symptoms in patients with sarcoidosis-associated small nerve fiber loss and increases corneal nerve fiber density. Mol Med. 2013;19(1):334-45., in the references below).
Araim Pharmaceuticals, its developer, had scientists on at least three of the four papers. ARA-290 is not FDA-approved and is sold online as a research chemical.
3. LL-37: tested on wounds, and it did not lower inflammatory markers
Small or uncontrolled human studies. Too little to settle the question either way. Four trials have given LL-37 to people: a topical solution on leg ulcers, a cream on diabetic foot ulcers, and an oral engineered-bacteria product in COVID-19. None tested an injection. Most of the LL-37 literature measures the body's own levels.
LL-37 is an antimicrobial peptide the body makes in skin, the gut and airway linings, and white blood cells (Dürr 2006 - Dürr UH, Sudheendra US, Ramamoorthy A. LL-37, the only human member of the cathelicidin family of antimicrobial peptides. Biochim Biophys Acta. 2006;1758(9):1408-25., in the references below). Most research on it measures people’s own LL-37 levels. Only a few trials have given it as a treatment:
- Venous leg ulcers, 34 people. A topical solution healed ulcers faster than placebo at the lowest dose. The highest dose did no better than placebo (Grönberg 2014 - Grönberg A, Mahlapuu M, Ståhle M et al. Treatment with LL-37 is safe and effective in enhancing healing of hard-to-heal venous leg ulcers: a randomized, placebo-controlled clinical trial. Wound Repair Regen. 2014;22(5):613-21., in the references below).
- Venous leg ulcers, 148 people. The follow-up trial, run by its developer Promore Pharma, found no significant improvement across the whole group (Mahlapuu 2021 - Mahlapuu M, Sidorowicz A, Mikosinski J et al. Evaluation of LL-37 in healing of hard-to-heal venous leg ulcers: A multicentric prospective randomized placebo-controlled clinical trial. Wound Repair Regen. 2021;29(6):938-950., in the references below).
- Diabetic foot ulcers. An LL-37 cream improved a measure of healing but did not lower the inflammatory markers IL-1α and TNF-α (Miranda 2023 - Miranda E, Bramono K, Yunir E et al. Efficacy of LL-37 cream in enhancing healing of diabetic foot ulcer: a randomized double-blind controlled trial. Arch Dermatol Res. 2023;315(9):2623-2633., in the references below).
- COVID-19, 238 inpatients. An oral product made by bacteria engineered to produce LL-37 shortened virus shedding (Zhao 2023 - Zhao Y, Zhang H, Zhao Z et al. Efficacy and safety of Oral LL-37 against the Omicron BA.5.1.3 variant of SARS-COV-2: A randomized trial. J Med Virol. 2023;95(8):e29035., in the references below).
FDA’s compounding safety page says it lacks the safety information to know whether LL-37 would cause harm in people. It flags possible immune reactions and non-clinical findings suggesting harm to male reproduction.
4. BPC-157: protective in rats, no significant effect in its one colitis trial
Small or uncontrolled human studies. Too little to settle the question either way. Five small published human studies, with about 80 people given BPC-157. The only placebo-controlled trial in patients, 53 people with ulcerative colitis, found no statistically significant difference and was published only as a meeting abstract.
Animal studies describe BPC-157 as protective in models of tissue injury and inflammatory bowel disease (Józwiak 2025 - Józwiak M, Bauer M, Kamysz W et al. Multifunctionality and Possible Medical Application of the BPC 157 Peptide-Literature and Patent Review. Pharmaceuticals (Basel). 2025;18(2)., in the references below). FDA’s July 2026 briefing lists five published human studies, with about 80 people given BPC-157.
The test that matters most for inflammation is a trial in ulcerative colitis, an inflammatory bowel disease. In 53 people with mild to moderate disease, BPC-157 or placebo was given as an enema once a day for two weeks. Disease scores fell more on BPC-157, but the difference between groups was not statistically significant. The trial was only ever published as a meeting abstract.
BPC-157 is not FDA-approved and is sold as a research chemical. On 23 July 2026, FDA’s compounding advisory committee voted 8-6, with one abstention, to recommend letting pharmacies compound it. As of 3 October 2026, I found no rule in the Federal Register adding it to the compounding list. For its joint evidence, see peptides for joint pain.
5. KPV: mouse colitis studies only
Studied in cells or animals. No controlled trials in humans. Cell and mouse studies only. FDA's July 2026 review found no clinical study or human exposure data for KPV by any route.
KPV is a three-amino-acid fragment (lysine-proline-valine) of alpha-MSH, a hormone with anti-inflammatory effects. In mice, KPV added to drinking water reduced chemically induced colitis and inflammatory cytokines (Dalmasso 2008 - Dalmasso G, Charrier-Hisamuddin L, Nguyen HT et al. PepT1-mediated tripeptide KPV uptake reduces intestinal inflammation. Gastroenterology. 2008;134(1):166-78., in the references below). A second group found it sped recovery and reduced colon inflammation in two mouse colitis models (Kannengiesser 2008 - Kannengiesser K, Maaser C, Heidemann J et al. Melanocortin-derived tripeptide KPV has anti-inflammatory potential in murine models of inflammatory bowel disease. Inflamm Bowel Dis. 2008;14(3):324-31., in the references below).
FDA’s July 2026 review found no clinical study or human exposure data for KPV by any route. The same 8-6 committee vote recommended KPV for pharmacy compounding, and I found no rule adding it either.
6. TB-500: no human data
Studied in cells or animals. No controlled trials in humans. FDA's July 2026 review found no clinical study or human exposure data for TB-500 by any route.
TB-500 is sold for inflammation and injury on the back of research into thymosin beta-4. It is only a synthetic seven-amino-acid fragment of that protein (Esposito 2012 - Esposito S, Deventer K, Goeman J et al. Synthesis and characterization of the N-terminal acetylated 17-23 fragment of thymosin beta 4 identified in TB-500, a product suspected to possess doping potential. Drug Test Anal. 2012;4(9):733-8., in the references below). FDA found no clinical study or human exposure data for TB-500. Its animal evidence is mostly tendon healing in rats (Biçer 2026 - Biçer O, Adanir O, Güleryüz Y et al. Effects of BPC-157 and TB-500 on Achilles tendon healing in rats: A histopathological and biomechanical study. Jt Dis Relat Surg. 2026;37(3):822-837., in the references below). The committee vote covered TB-500 too.
Also sold for inflammation
- GHK-Cu. A cosmetic copper peptide. Its human studies are small and topical, mostly on skin. See copper peptides.
- VIP. Sold as a research peptide for immune modulation. I could not verify a completed human trial for that use. See VIP.

Common questions
Is BPC-157 an anti-inflammatory?
In animal and lab studies it is described as protective in injury and inflammatory bowel disease models. In people that has not been shown. The one placebo-controlled trial gave 53 people with ulcerative colitis BPC-157 or placebo enemas for two weeks. It found no statistically significant difference and was published only as a meeting abstract.
What is the strongest peptide for inflammation?
By volume of human evidence, thymosin alpha-1. PubMed tags 64 of its records as randomised trials. Those trials measured outcomes such as sepsis deaths, pancreatic infection and hepatitis B response rather than everyday inflammation. The largest, in 1,106 people with sepsis, found no clear survival benefit.
What is the best anti-inflammatory peptide stack?
No stack has been tested in people. The common ones are KLOW (GHK-Cu, BPC-157, TB-500 and KPV) and BPC-157 with KPV, and I found no human or animal study of either combination. The only study I found that combined two of these peptides used BPC-157 and TB-500 in rats with repaired Achilles tendons. The pair did no better than either alone.
What is the most powerful supplement for inflammation?
I found no trial that answers this, because supplements are rarely compared head to head. For osteoarthritis pain, a review of 69 trials found large short-term effects for a few supplements, including collagen hydrolysate and extracts of turmeric and Boswellia. It rated the overall evidence very low quality. None of the supplements had a clinically important effect on pain in the long term.
What peptides does Joe Rogan take?
Joe Rogan said on his podcast in July 2021 (episode 1683) that elbow tendonitis he could not fix was gone within two weeks of starting BPC-157. The Associated Press reported the same account in November 2025. In September 2021, CBS News and Variety reported, he said he had taken an NAD drip while ill with COVID-19. NAD is not a peptide. That is one person's account, not evidence of what BPC-157 does.
Do oral peptides work for inflammation?
Oral KPV is sold on the strength of mouse studies in which KPV added to drinking water reduced chemically induced colitis. FDA found no human exposure data for KPV by any route, so no one has tested whether swallowed KPV does anything in people. An oral LL-37 product shortened virus shedding in a COVID-19 trial. It was a bacterium engineered to make LL-37, not a capsule of the peptide.
Is thymosin alpha-1 FDA-approved?
No. FDA's December 2024 review states it is not approved in the United States, Japan or Europe, except Italy. SciClone Pharmaceuticals, which makes it as Zadaxin, said in its 2014 annual report that it was approved in the Asia-Pacific region, Latin America, Eastern Europe and the Middle East. FDA said it could not verify those approvals in every country.
