Thymosin Alpha-1: A Real Medicine, Tested in People Far Sicker Than You

64 randomised trials and approvals outside the US. The newest and largest, in the BMJ, found no clear survival benefit. None of it studied healthy people.

Thymosin alpha-1 is an approved medicine in a number of countries, though not in the United States, and it has a large clinical trial record. Those trials are in sepsis, pancreatitis and hepatitis - hospital patients, not healthy adults. The newest and largest, published in the BMJ in 2025, found no clear survival benefit.

Thymosin alpha-1 has more clinical evidence behind it than anything else we cover in the research-chemical market. Eight hundred and sixty-four papers. Sixty-four randomised controlled trials. An international non-proprietary name, thymalfasin, and marketing approval in a number of countries.

It is a real medicine. It has also never been tested for the thing it is sold for online, and its newest and largest trial is negative.

The clinical programme is in hospital medicine: sepsis, severe pancreatitis, chronic hepatitis. Nobody has run a randomised trial of thymosin alpha-1 for general immune support in healthy adults, which is what the online market sells it for.

The newest trial, and what it found

TESTS was a multicentre randomised controlled trial asking whether thymosin alpha-1 reduces mortality in adults with sepsis, published in the BMJ in 2025 Wu 2025 - Wu J, Pei F, Zhou L et al. The efficacy and safety of thymosin α1 for sepsis (TESTS): multicentre, double blinded, randomised, placebo controlled, phase 3 trial. BMJ. 2025;388:e082583. (opens PubMed in a new tab). Its primary outcome was 28-day all-cause mortality.

Its conclusion, in its own words: the trial “found no clear evidence to suggest that thymosin α1 decreases 28 day all cause mortality in adults with sepsis.” No secondary or safety outcome differed significantly between groups either.

The gap between the evidence and the pitch

Read the settings again: sepsis. Necrotising pancreatitis. Chronic hepatitis.

These are people in hospital, often in intensive care, whose immune systems are failing under enormous acute stress. Thymosin alpha-1 is being investigated as a way to modulate that.

It is sold online to healthy adults who would like to get fewer colds. There is no trial of that, and the trials that do exist do not transfer to it — a drug that fails to reduce mortality in sepsis has told you nothing about your winter.

Thymosin alpha-1 is not FDA-approved. It holds marketing approval in a number of other countries, where it is a licensed prescription medicine rather than a research chemical. We are verifying the specific national approvals against primary regulators before naming them here.

This is the compound that most clearly justifies recording legal status country by country rather than as one global fact. “Thymosin alpha-1 is an approved medicine” and “thymosin alpha-1 is not approved” are both true statements, and which one governs you is a matter of geography.
